Advance Care Planning

I am available to meet with you and your family to clarify your end-of-life goals, understand your options and document your wishes. I can assist with documents including Advanced Health Care Directives and Physician’s Orders for Life Sustaining Treatment; help navigate the California End of Life Options Act (or Medical Aid in Dying); support around final plans including burial, cremation and body donation; and offer referrals to community support services in the Bay Area for added in-home and community support.
